School-Based Health Clinics

Thanks to our community partners at Community Clinic & Mercy Health System, access to family medical care has never been more convenient!

Parents do not have to miss work and students do not have to miss valuable class time. Some of the services provided at our school-based health clinics include:

  • Non-emergency care visits - care for illness and injury

  • Basic lab tests (strep, flu, blood, etc...)

  • Wellness checks, Sports Physicals, and Comprehensive Annual Exams

  • Immunizations for children who have ARKids or are uninsured

  • Follow up visits and referrals to specialty providers

  • Transportation to/from the clinic is available for all students and their parents/guardians

The Wellness Centers do not provide emergency services.

Who can use The Wellness Centers?

All Springdale Public School students, staff, and their family members are welcome at any of the Wellness Centers.

How do I enroll myself or a child at The Wellness Centers?

You are encouraged to enroll online here for Community Clinic OR here for Mercy Clinic.

You may find the paper version of the forms at:

  • The Wellness Centers

  • The school nurse in every school

What about insurance and payment options?

Medicaid, Medicare, and most private insurance plans are accepted. We will file with your insurance, however, a co-pay may be requested. A sliding scale fee discount is available for patients who qualify based on income. No child will be turned away because a family cannot pay.

If you need assistance with applying for health care coverage, including Medicaid/ARKids, please contact an Enrollment Specialist at Community Clinic for an appointment at (479) 256-5080. The staff at the Wellness Centers can also assist with Medicaid applications.

How do I make an appointment?

Call The Wellness Center of your choice using the contact information above, or contact your student's school nurse for assistance.

How do I schedule transportation?

If you need transportation to your appointment, your student's school nurse can arrange it for you. Transportation is not limited to medical appointments at the Wellness Centers.

Transportation may be available for students who are going to the dentist, eye doctor, and other medical appointments. Please contact your school nurse for more information.
